You have probably seen this figure cited somewhere in corporate wellness marketing. Maybe on a vendor slide deck. Maybe in an HR conference. Maybe a well-meaning people manager dropped it into a budget proposal.
For every $1 invested in employee wellness, companies see a $3 return.
Maybe you believed it. Maybe you dismissed it immediately. I am not going to tell you which reaction was correct — at least not yet.
What I am going to do instead is lay out the evidence as honestly as I can. Not the vendor-funded kind. The peer-reviewed, mechanism-level kind that does not care whether you buy anything at the end.
Let's see what the evidence actually says.
The Cost You Are Not Tracking
Every business owner and team lead I have spoken to tracks the same things: sick days, headcount, delivery velocity, sprint completion, Net Promoter Score. These are the visible costs. The ones that show up on reports.
There is another cost that does not show up anywhere. Researchers call it presenteeism — the productivity loss that occurs not when your people are absent, but when they are present and operating below their capability.
This is the engineer who is physically at his desk but mentally running at 60% because he slept badly for the third consecutive week. The team lead who is in the standup but whose decision quality has been quietly degrading under chronic stress load for months. The senior developer who ships, technically, but whose problem-solving depth has contracted without anyone formally registering it.
A peer-reviewed analysis published in the Journal of Occupational and Environmental Medicine found that lost productivity costs from presenteeism run approximately 2.3 times higher than direct medical and pharmacy costs combined.1 Not slightly higher. More than double. A separate large-scale study confirmed the same pattern: presenteeism costs dwarf absenteeism costs, and account for the largest proportion of total health-related costs in organisations.2
The reason this number never appears on your dashboard is structural. Absenteeism generates a record — a sick day is a data point. Presenteeism generates nothing. It is invisible by default, buried in slightly missed deadlines, decisions that were good enough but not sharp, and the slow accumulation of mediocre output that nobody can point to specifically but everyone dimly senses.
Your team almost certainly has a presenteeism problem. You almost certainly cannot see it. That is not a management failure — it is a measurement failure. The cost is real regardless of whether it is captured.
What Is Actually Causing It
Presenteeism has many faces: mental health conditions, musculoskeletal pain, sleep disorders, chronic fatigue. The Japanese study cited above found that neck and shoulder pain alone costs approximately $28 billion annually in lost productivity across the country's workforce, with mental illness adding another $31.7 billion.2 These are not exotic conditions. They are the predictable physiological consequences of sedentary, high-stress, screen-heavy knowledge work — the exact conditions your engineering team operates in every day.
The common thread running through all of them is physical. Not psychological. Not motivational. Physiological.
Your people are sitting for eight to twelve hours a day. They are under sustained cognitive and emotional load. They are sleeping poorly because their nervous systems never fully downregulate. They are eating inconsistently because deadlines compress the day. Their cardiovascular systems are deconditioned — which means less oxygen reaching the brain you are paying them to use. Their bodies have adapted, over years, to a mode of functioning that is technically survivable but nowhere near optimal.
And here is the part that matters for you as a leader: the cognitive faculties that degrade first under these conditions are not the basic ones. They are the executive ones. The ones you are paying senior salaries for.
The Cognitive Faculties You Are Paying For — and What Erodes Them
When researchers study the relationship between physical fitness and brain function, they keep arriving at the same finding: cardiorespiratory fitness is most strongly associated with executive function.3 4
Executive function is not a vague term. It refers to a specific cluster of higher-order cognitive processes: inhibitory control — the ability to suppress an irrelevant response and choose the correct one. Working memory — the ability to hold and manipulate information in real time. Cognitive flexibility — the ability to shift between mental sets, update a model when new information arrives, and switch approaches when the current one is not working. Planning and problem decomposition.5
This is the cognitive profile of a senior engineer, a technical lead, or an architect. It is not the profile of someone doing routine data entry. It is the profile of someone whose entire professional value lies in the quality of their judgment and the speed and accuracy of their thinking under pressure.
The mechanism is direct and well-characterised. Higher cardiorespiratory fitness maintains cerebral blood flow and vessel elasticity. It preserves brain volume in frontal and temporal regions — precisely the regions responsible for executive control. It modulates oxygenation in the prefrontal cortex, where executive function is processed.3 When fitness declines, these systems degrade. Not catastrophically and not overnight — but measurably, progressively, and silently.
Your deconditioned senior engineer is not the same cognitive instrument he was five years ago. He may not know it. You may not be able to see it. But the substrate has changed.
A landmark study by Coulson, McKenna, and Field at the University of Bristol put numbers on the acute version of this effect.6 In a randomised crossover trial with white-collar workers, employees were measured on exercise days versus non-exercise days. On days they exercised, concentration was 21% higher. The ability to work without unscheduled interruptions was 25% higher. The ability to finish work on time was 22% higher. These were the same people, the same jobs, the same workload — the only variable was whether they had moved their bodies that day.
This is not a wellness claim. This is a performance measurement.
Now connect it to the presenteeism cost established earlier and do the arithmetic yourself. Your senior engineers are not exercising on most days. Their cognitive performance is not at its ceiling on most days. The Coulson numbers give you a floor estimate of the gap between what you are paying for and what you are actually receiving. And the presenteeism research tells you that this gap — invisible, unmeasured, never appearing on a dashboard — is running at more than double the cost of your entire medical and pharmacy spend.
Whatever your senior engineers cost per hour, ask what percentage of that value is actually being delivered on a typical Tuesday afternoon, three hours after lunch, in week two of a demanding sprint. Then ask what that same engineer delivers on a day when his body is fuelled, recovered, and moving. The research gives you the direction. The magnitude you can calculate against your own payroll.
The Team Problem That Fitness Also Solves
So far I have been talking about individual performance. But you are not managing a collection of individuals. You are managing a team — and teams have a property that individuals do not: trust.
Google's Project Aristotle, one of the most rigorous internal studies ever conducted on team performance, identified psychological safety as the single strongest predictor of team effectiveness.7 Not technical skill distribution. Not process quality. Not tooling. The degree to which team members feel safe to take interpersonal risks — to disagree, to admit uncertainty, to share a half-formed idea — determines more of the team's output than any other variable.
Psychological safety is not built in retrospectives. It is not built in team-building workshops. It is built through shared experience, mutual vulnerability, and the kind of connection that only emerges when people have been through something real together.
This is where shared effort — not shared office space, not shared tooling, but a genuinely shared physical and personal challenge — becomes a tool that nothing else in your management toolkit replicates.
The mechanism does not require your team to exercise in the same room. It requires them to be going through the same experience at the same time, with a place to surface it together. When people are each facing the same discomfort, making the same commitments, and showing up to a weekly conversation where they can say honestly how it is going — something shifts in how they relate to each other. The vulnerability that emerges from a shared physical challenge is different in kind from the vulnerability of a retrospective or a one-on-one with a manager. It is personal. It is equal. Nobody has more authority over their body than anyone else on the team.
The neuroscience supports this. Shared challenge triggers oxytocin release — the neurochemical most directly associated with trust, social bonding, and cohesion.8 9 Mirror neurons fire powerfully when we know others are in the same effort we are in, creating alignment and genuine empathy that no team-building workshop produces.10 These are not metaphors. They are measurable biological responses to a specific social condition: people who perceive themselves as going through something real together.
The trust built through that shared journey carries directly into the work. The willingness to be honest about a difficult physical week transfers into the willingness to say "I do not know" in an architecture review. Teams that have seen each other as full human beings — not just role-holders in a Slack workspace — collaborate differently. They share ideas earlier, disagree more productively, and cover for each other under pressure without being asked.
What the People I Have Coached Report
I have been running this system with individual operators — leaders, managers, and senior professionals across industries. These are the people who went through exactly the protocol I now bring to teams.

These are not exceptional cases. They are predictable outcomes of applying a structured, science-backed protocol to a person who was previously operating without one. Five weeks to a visible physical shift. Eight weeks to a sustainable energy system that no longer requires caffeine and willpower to maintain.
The physiological change is real. The performance implication is direct.
Why the Gym Membership and the Insurance Are Not Enough
Most companies I have spoken to feel they have already addressed this. They point to the health insurance. The gym subsidy. The wellness stipend. The occasional yoga session or step challenge.
I understand the instinct. These are genuine investments and they come from a real desire to take care of people. But access is not outcome. Offering a gym membership to an engineering team that has no time, no structure, and no accountability system is roughly equivalent to offering a library card to someone who does not yet know how to read. The resource exists. The capacity to use it does not.
Generic wellness programmes fail for a specific reason: they are built for average employees with average schedules. Your engineering team does not have an average schedule. They have irregular hours, on-call rotations, deadline-driven weeks where everything else evaporates, travel, and the cognitive demands of sustained deep work. A generic step challenge does not fit into that life. A protocol built specifically around how they actually live does.

That is a measurable physiological shift in a person who was previously doing nothing differently except following a structured protocol. HRV is not a vanity metric. It is one of the most sensitive indicators of nervous system recovery and stress load that consumer wearables can capture. A jump from 32 to 39 in a matter of weeks means the body is recovering more completely between efforts. That recovery capacity shows up directly in next-day cognitive performance, emotional regulation, and decision quality.
So — Is the $3 Number True?
Here is my honest answer: I do not know. And I am suspicious of anyone who tells you they do.
The $3 figure comes from a category of studies that are genuinely difficult to design well. Isolating the return on a wellness programme from every other variable affecting a business over the same period — economic conditions, product changes, management transitions, team composition — is a methodological challenge that most studies do not fully solve. The most rigorous large-scale randomised trial of workplace wellness programmes, conducted at the University of Illinois, found no significant effect on health outcomes or medical spending in the first year.11 That does not mean wellness programmes do not work. It means that the packaged claim of a guaranteed 3-to-1 return is not as solid as the slide deck makes it look.
But here is what I know with considerably more confidence.
Your team's cognitive performance is directly tied to their physical condition. The research on this is not contested. The mechanisms are understood. The effect is measurable at the individual level and it is substantial.
Your team's trust and cohesion — the psychological safety that Google identified as the strongest predictor of team effectiveness — is built through shared experience and vulnerability. Shared physical effort is one of the most reliable ways to create both. This is also not contested.
The costs of not addressing this — the presenteeism, the gradual erosion of executive function, the invisible friction in team collaboration — are real and ongoing, even if they never show up on a report you look at.
And the investment required to change this, at least at the pilot scale I am proposing, is genuinely small relative to what a senior engineering team costs per month in salaries alone.
So does the number matter? Does it matter whether it is $3 or $1.50 or $4.20?
If your team's physiology is the bottleneck on your team's output — and the evidence strongly suggests it is — then the direction of the return is beyond reasonable doubt. The precise magnitude is a measurement problem. The existence of the return is not.
